QuickScript Pharmacy, a NY State providers enrolled in the Registry are not required to complete the Vision Test Report (MV-619) or (MV-619R) for a vision test customer. Instead, the provider or their authorized personnel can enter vision test results online. Customers whose results are entered into the Vision Registry can then renew their driver licenses online or by mail without a paper vision test form.
